Origin Wellness Affiliate Agreement

Between: Origin Wellness ("Company") and the registering Affiliate ("Affiliate")

1. Program Overview

Origin Wellness is a medically supervised telehealth platform offering compounded GLP-1 medications (Tirzepatide and Semaglutide) as well as premium non-GLP-1 peptides. Every program includes access to diagnostic lab testing and oversight by licensed medical providers.

2. Compensation & Discounts

A. Affiliate Commission

You will earn a 5% commission on every successful paid referral. A successful referral is defined as a new customer who completes a paid program using your unique affiliate link or code and does not request a refund or initiate a chargeback.

Commissions are paid monthly via ACH.

B. Customer Discounts

All first-time customers who use your affiliate link or code receive $35 off their first program. This discount may be promoted to your audience.

C. Affiliate Personal Discount (Wholesale Pricing)

Approved affiliates receive 10% off all personal orders placed directly with Origin Wellness.

This discount is strictly for your personal use only. It may not be shared publicly, used for family or friends, or promoted to your audience. The discount code is tied exclusively to your affiliate-tagged account.

3. Tracking, Payouts & Order Policy

Upon approval, you will receive a unique coupon code and access to your real-time dashboard through Affiliatly for tracking referrals, orders, and earnings.

Important Order Policy: Client orders cannot be modified after they are completed through the online checkout process. Discounts cannot be applied retroactively, and commissions cannot be added to orders where the affiliate code was not used at the time of purchase. It is the Affiliate's responsibility to ensure clients use their unique code.

Commission reports are updated monthly. Orders placed during the current month may not appear in your dashboard until the next reporting cycle.

4. Affiliate Obligations & Compliance

As an Affiliate, you agree to the following:

Clearly disclose your affiliate relationship in all promotions using #ad or #affiliate (or equivalent clear disclosure).
You may not prescribe medications, recommend specific treatments, make medical claims, or guarantee results.
You may not present yourself as an employee, representative, or licensed provider of Origin Wellness.
All promotional content must comply with the Approved Messaging Guide located in the FAQs of your Affiliatly dashboard.

5. Term & Termination

This is a month-to-month agreement. Either party may terminate the agreement with 30 days' written notice or immediately in the event of a breach.

Commissions earned on referrals completed prior to termination will still be paid according to the terms of this agreement.

6. Intellectual Property, Confidentiality & Indemnification

You are granted a limited, non-exclusive, non-transferable license to use Origin Wellness trademarks and approved marketing materials solely for promotional purposes.

You agree to indemnify and hold harmless Origin Wellness from any claims, damages, or expenses arising from your marketing activities or breach of this agreement.

7. Governing Law

This agreement shall be governed by and construed in accordance with the laws of the State of Texas.

By joining the Origin Wellness Affiliate Program and clicking to accept, you acknowledge that you have read, understood, and agree to be bound by the terms of this Affiliate Agreement.
